Accessing your medical record



How to view your medical record

The first step to take is to register to use your NHS account.

The entire registration process (including proving your identity) is done online with no need to go to your GP surgery.

You can also access your NHS account via the NHS App and use it on your smartphone or tablet.

Using your NHS account

Once registered, your NHS account allows you to do a number of things, including:

  • Check your symptoms to get health advice
  • Find out what to do when you need help urgently
  • Book & manage appointments at the surgery
  • Request repeat prescriptions and change your nominated pharmacy
  • View recent entries from your GP record securely
  • Register to be an organ donor
  • Choose how the NHS uses your data.

There are a number of other third-party apps that you can use to interact with your GP surgery but not all of these are compatible with the NHS logon.

If you are over 16 and have an online account, most patients are now able to see all new notes & health records from The Corner Surgery automatically.  This also includes results of tests that we have requested & hospital letters that we have received.  A clinician may need to talk to you to discuss test results before you are able to see this information online.  One of our GPs may also need to talk to you before your full records access is given to make sure that having access is of benefit to you.  There might be some sensitive information in your record, so you should contact the surgery if you have any concerns.  If you do not want to see your GP medical record online please contact our reception.

For the time-being, the surgery does not provide the option for patients to request access to their historic GP medical record.  This would require one of our GPs to screen the patient's record for sensitive, harmful or third-party information, which would reduce our time available to offer GP appointments for medical problems.  In addition, the surgery does not offer proxy online access, whereby somebody other than the patient is able to access a patient’s online services.

Updating your personal details

It is important to let us know straight away if any of your personal details change, especially your:

  • Name
  • Address
  • Telephone number
  • Mobile number
  • Email address 

You can change your mobile phone number and email address using the NHS app or one of the other third-party apps if they have the functionality.

If you inform us that you have moved house and your new address is outside of our published practice boundary, you will need to register with a GP nearer your new home.  We will commence the process when we receive your notification and this will give you 28 days to register with a new practice.  If you don't register within this time period, your medical record will not be transferred in digital format to your new practice and this might disrupt the continuity of your care.

Subject Access Requests (SARs)

To request a full copy of your GP medical record, please complete our Access Request Form and send it to the surgery.  There is no charge for this unless you have been provided with the information previously.
