We are constantly seeking to improve the range of services to our patients and currently offer:
- Doctor and advanced nurse practitioner consultations
- Extended hours appointments - via the Primary Care Network
- Long term condition reviews for:
- Coronary heart disease
- Heart failure
- High blood pressure
- Diabetes
- Kidney disease
- Previous strokes
- Dementia
- Depression
- Severe mental illness
- Epilepsy
- Asthma
- Chronic obstructive pulmonary disease
- Learning disability health checks
- Weight management referrals
- Cervical screening (smear tests)
- Contraceptive services
- Maternity services - in conjunction with local midwives
- Child health surveillance
- Certain vaccinations, immunisations and injections
- Certain joint injections - only at the instigation of one of our GPs
- Certain minor surgery procedures
- Phlebotomy (taking blood samples)
- Ankle brachial pressure index (circulation) tests
- Monitoring of certain higher risk drugs
- Social prescribing link worker assessments
- Structured medication reviews - via the medicines management team
- Mental health assessments - via our mental health practitioner
- Cancer care support - via our cancer navigator
