Request a fit note (sick note) for work

You do not require a doctor's sickness certificate for any illness lasting seven days or less. However, your employer may require you to complete a self-certification form (SC2) which is available from your employer or via the GOV.UK website
If you have been off work for more than seven days or believe you are likely to be, and your problem has already been assessed by one of our clinicians, please request a fit note online using the link below:
If you are unable to use the eConsult online consultation tool to request a fit note then please contact the surgery by phone.
We do not provide fit notes on behalf of hospitals when it is they who have assessed your problem - e.g. emergency departments, outpatient clinics or following an operation. Please contact the hospital instead.
We do not provide fit notes for young people who have missed school/ college/ university due to an illness or injury. If this is required, we would charge a fee.
